My child will thoroughly wash or sanitize hands before each lesson

I will cancel the lesson if my child or any family member experiences any symptoms of Covid-19 or has a temperature over 100° fahrenheit. I will also cancel the lesson in anyone in my household has been in close contact with someone diagnosed with Covid-19 in the last 14 days

I undertand that the instructor will wash or sanitize their hands prior to each lesson. They will wear a mask if a social distance of 6ft cannot be maintained. A teacher may request the student to wear a mask at any time. The teacher will cancel the lesson if experiencing any symptoms of Covid-19, has a temperature over 100° fahrenheit, or has been in contact with anyone diagnosed with Covid-19 within in the last 14 days.
